An irony in safety is that many organizations with exemplary safety records have a higher risk of a catastrophic failure than do organizations that are actively working to improve their records. Complacency can accompany high safety performance and result in few reinforcers for monitoring behavior. Therefore, when an incident does occur, it has the potential to be catastrophic; no one will be looking because looking will likely have undergone extinction.
